Signature IV – Mobile IV Therapy Sydney

Welcome to Signature IV. By booking an appointment or using our services, you agree to the following Terms & Conditions. Please read them carefully.

1. Services

Signature IV provides mobile intravenous (IV) vitamin therapy services within Sydney, Australia. All treatments are administered by qualified healthcare professionals in accordance with Australian health regulations.

Our services are wellness-based and are not intended to diagnose, treat, cure, or prevent any disease. IV therapy is not a substitute for medical care provided by your general practitioner or specialist.

2. Mandatory Scripting Fee

A $50 mandatory scripting fee applies to all new clients at their first appointment.

• This fee covers the required medical consultation and prescription.

• The scripting fee is valid for 12 months from the date of issue.

• After 12 months, a new consultation and scripting fee will be required to continue treatments.

• This fee is payable at the first appointment and is separate from the IV treatment cost

3. Booking & Deposits

To secure a mobile appointment:

• A $100 deposit is required at the time of booking.

• The deposit will be deducted from the final treatment total.

• An appointment is not confirmed until the deposit has been received.

To secure your booking, we require personal details including (but not limited to):

• Full name

• Date of birth

• Residential address

• Contact details

• Relevant medical history

These details are required to create a client profile and ensure safe administration of treatment.

4. Cancellation & Rescheduling Policy

We understand that plans change. However, due to the nature of mobile services:

• Cancellations made within 24 hours of the scheduled appointment will result in the $100 deposit being forfeited.

• Cancellations made more than 24 hours prior may transfer the deposit to a future booking.

• Rescheduling requests must be made at least 24 hours prior to your appointment.

• You may reschedule by calling, emailing, or messaging us directly.

Failure to attend your appointment without notice will result in loss of your deposit.

5. Call-Out Fees

Signature IV is a mobile service operating across Sydney.

• Additional call-out fees may apply depending on your location.

• Please contact us prior to booking to confirm whether your suburb incurs an additional travel fee.

6. Client Eligibility & Medical Clearance

All clients must:

• Be aged 16 years or older (unless accompanied by a legal guardian and medically approved).

• Complete a medical screening questionnaire.

• Provide accurate and truthful health information.

Treatment is subject to clinical assessment and approval. Signature IV reserves the right to decline treatment if deemed medically unsuitable or unsafe.

7. Treatment Risks & Consent

While IV therapy is generally well tolerated, potential risks may include (but are not limited to):

• Bruising or discomfort at injection site

• Infection

• Allergic reaction

• Dizziness or fainting

By proceeding with treatment, clients acknowledge that they have been informed of the potential risks and provide voluntary consent.

8. Payment Terms

• Full payment is required at the time of service.

• We accept approved electronic payment methods.

• Prices are subject to change without notice.

9. Refund Policy

Due to the medical nature of our services:

• No refunds are provided once treatment has been administered.

• Deposits are non-refundable if cancellation occurs within 24 hours.

• Refunds are not provided for change of mind.

10. Privacy & Confidentiality

Signature IV is committed to protecting your privacy.

All personal and medical information collected is handled in accordance with the Privacy Act 1988 (Cth) and relevant Australian privacy laws. Information is used solely for medical assessment, treatment, and internal record-keeping.

We do not share personal information with third parties without consent unless required by law.

11. Limitation of Liability

To the extent permitted by law, Signature IV shall not be liable for:

• Any indirect or consequential loss

• Any adverse reaction where full medical history was not disclosed

• Outcomes that vary between individuals

Results may differ from person to person.

12. Changes to Terms

Signature IV reserves the right to update or modify these Terms & Conditions at any time. The latest version will always be available on our website.
